Nomination Form for Arizona's 2025 Secondary Transition Services Awards
The Arizona Department of Education/Exceptional Student Services welcomes your nominations for the 2025 Secondary Transition Services Awards! The Secondary Transition Services Awards recognize individuals for their excellence in providing transition services and their successful navigation through the transition planning process toward postsecondary adulthood.
An appropriate nominee is one who is committed to positively impacting postschool outcomes for him or herself or for youths that he or she serves. When appropriate, the nominee also continuously strives to improve his or her professional practices by implementing strategies that go beyond compliance and support best practice. This individual's dedication to improving transition planning exceeds his or her basic job description or the general aspects required to prepare for adulthood.
Nomination Categories:
Youth/Young Adult: A youth or recently graduated young adult with active engagement and successful navigation of the secondary transition planning process.
Community Member: An individual (employee, manager, owner of a community business, etc.) who provides exemplary support for students and/or young adults with disabilities to enhance community-based experiences and opportunities.
Educator: A PEA or Secure Care certified staff, classified staff, support staff member, related service provider, or administrator who provides remarkable secondary transition services.
